2011 MEMBERSHIP APPLICATION
Being desirous of acceptance in the Krewe of Dionysus, I hereby submit this, my application, and if accepted, agree to abide by the Krewe Rules, Regulations and By-Laws as formulated by the Board of Directors.
I understand and agree to abide the Krewe Rules, Regulations and By-Laws for application to the Krewe
1. The Annual dues are $400.00 and due upon acceptance to Krewe
- Payment plan is available and will be as follows:
Three installments
First payment of $150.00 due March 30
Second payment of $125.00 due June 15
Third payment of $125.00 due August 10
2. The Annual dues includes the following Activities:
a. Seven General Membership meetings, member only.
b. Family Picnic, member & spouse or date and their children.
c. Pre-Parade party, member & spouse or date
d. Breakfast Day of Parade, member only.
e. Krewe Costume, member only.
f. Position on Float Parade Day, member only
3. Krewe’s Annual Coronation Ball will be by reservation only; a $100.00 fee will be required for attendance. This fee is non-refundable, member & spouse or date.
4. Student Rider must be between the ages of 16-18 or senior in high school, and must ride next to the sponsoring member, fee for a student rider is $300.00 (Parade and Costume Only)
5. Should I resign from the Krewe, I must notify the Board of Directors by May 1, other wise I remain responsible for DUES payments as they become due.
